Reporting to the designated manager, the Social Worker provides psychosocial assessment, supportive counselling, case management, discharge planning, and service coordination to support patient recovery and safe transitions through the care continuum. The social worker assists patients/clients and families in understanding treatment plans, navigating community resources, and addressing social determinants of health.
The role collaborates with interprofessional teams, advocates for patient needs, and builds strong community relationships to promote access to care and quality of life. The Social Worker adheres to the OCSWSSW Code of Ethics and Standards of Practice and supports organizational priorities related to access, flow, and transitions.
Education & Experience:
- Bachelor of Social Work (BSW) from a recognized educational institution
- Current unrestricted registration with the Ontario College of Social Workers and Social Services Workers as a Social Worker (RSW)
- Experience providing Social Work services in a hospital or related setting is preferred
- Experience with electronic patient record systems, preferred
Position Duties & Responsibilities:
- Prioritizes referrals, assessments and interventions based on urgency and identified domains
- Performs client assessment utilizing population-specific and evidence-based assessment strategies
- Provides crisis intervention, as necessary
- Uses evidence informed supportive strategies and engages and supports clients and families in establishing person-centered goals, including promotion of self-management skills.
- Uses a strength-based approach to support clients in achieving optimal health/functioning/recovery
- Advocates for, and arranges family/community support team conferences as necessary, in consultation with the patient/client
