Under the general direction of the Manager, Access to Care and Transitions, the Access to Care and Transitions Specialist facilitates safe and efficient admissions, transfers and discharges of patients/clients supporting best practice and optimized resource utilization. Providing screening and assessment of potential candidates  for admission, the  Specialist collaborates with team members, patients/clients and their families, other care providers and community partners to ensure appropriate and timely admissions, transfers, and discharges that enhance continuity of care and efficient use of hospital and community resources.  As an effective team member, the Specialist participates in profession-specific and inter-professional activities that are client-centered and contribute to quality and safety, and operational efficiency, while promoting professional and program development.

 

Skills and Qualifications:

  • Degree in Nursing required or an equivalent combination of relevant education and experience may be considered
  • Current, unrestricted license to practice with the College of Nurses of Ontario
  • Minimum 1 year of recent experience working in Intake and Discharge Planning preferred
  • Minimum 3 years’ experience in Nursing within the specified stream of care (Mental Health AND Complex Medical, Palliative, Gerontology and/or Rehabilitation).
  • Minimum 1 year experience working collaboratively with internal and external stakeholders involving patient flow, or Health system continuous improvement initiatives preferred.
  • Current Certification related to specified care stream preferred (CNA certification or other accredited college certificate)

 

Position Responsibilities:

  • Engages in activities aimed at identifying potential referrals and promoting the PCH services, in collaboration with acute care and community partners
  • Receives and assesses new referrals utilizing a comprehensive evidence-based framework and communicates findings/recommendations internally and externally to appropriate parties. 
  • Acts as resource for inpatient predictive discharge planning activities including providing input as to expected length of stay and by contributing to discharge planning through pre-admission discharge planning discussions with patient/client.
  • Liaises with South East LHIN Home and Community care partners on patient/family predictive discharge plans to facilitate discharge and sustain efficient and effective patient flow, as assigned
  • Provides information, education and support to patient/client and family regarding discharge and care options, as appropriate
  • Builds team understanding of role of Access to Care and Transitions Specialist and the operational priorities related to access to care and transitions for PCH
