The CCRS/NRS Coordinator is responsible for leading and supporting teams in valid and efficient data entry, and related data collection for patient assessment under both the National Rehabilitation Reporting System (NRS) and the Continuing Care Reporting System (CCRS). This role will ensure accuracy of data and distribution of data within mandated timelines.  Reporting to the Manager, Access to Care and Transitions, the CCRS/NRS Coordinator has primary responsibility for the assessment of Complex Continuing Care and Rehabilitation patients including the coordination of assessment timelines, collection of assessment information, coordinate assessment completion and complete appropriate sections of the CCRS and NRS. Working collaboratively with all members of the clinical teams, the CCRS/NRS Coordinator acts as a primary contact for CCRS and NRS data collection and is responsible for timely performance of interprofessional team member assessments for the CCRS and NRS.  The CCRS/NRS Coordinator supports nursing and allied health care professionals in timely and accurate documentation for data collection.  Working collaboratively with the Clinical Business Operations Coordinator and the Decision Support Performance Indicator Specialist, the CCRS/NRS Coordinator will ensure services have access to, and analysis of, any information available through the collection of patient assessments. The CCRS/NRS Coordinator will understand and promote the key drivers of funding as related to the CCRS and NRS data and identify and lead areas where further information/education development is required.

 

Skills and Qualifications Required:

  • Baccalaureate degree in Nursing preferred and current, unrestricted registration with the College of Nurses of Ontario
  • Minimum of three years of clinical experience in rehabilitation and/or complex care setting
  • Minimum of three years of experience in a health care setting using hospital systems
  • Recent experience in working with CCRS/NRS data or similar clinical data set from another sector, preferred
  • Experience with performance measurement and quality improvement methodologies, preferred
  • Experience in leadership and/or education position, preferred

 

Position Responsibilities:

  • Meets the mandated reporting requirements of the Ministry of Health and the CIHI
  • Coordination of all required NRS and CCRS assessments as per regulations and protocols to ensure timely completion
  • Assesses and completes appropriate sections of the CCRS and NRS assessments with the appropriate and related patient population
  • Participates in inter-professional team meetings including client goal review meetings
  • Educates allied health professionals and physicians about the completion and importance of CCRS and NRS assessments and processes and ensuring they have the needed support and resources
  • Development of resource materials and workshops for the interprofessional teams for both current and new employees
  • Act as a resource regarding utilization of the computer software programs which support the CCRS and NRS
  • Consults and collaborates with clinical teams and site experts to encourage and facilitate the clinical utilization of data
  • Consults and collaborates with Decision Support staff to encourage the use of data to enhance its application to clinical teams
