Reporting to the Director of Patient Flow, the Patient Flow Coordinator plays a key role between Providence Care's staff, physicians and external partners in ensuring seamless, quality and efficient patient flow activities. The Coordinator responds to inquiries from key stakeholders directly and/or indirectly and assists the Director by researching and providing requested data or background information, as requested. The Coordinator problem solves barriers to transitions in care by working closely with the Access to Care & Transitions Specialists. The Coordinator completes daily, weekly and monthly summaries provided to relevant 'leaders in the hospital and is fully informed of issues and challenges that require action.  Utilizing sound clinical judgment and strong assessment skills, the Patient Flow Coordinator screens and evaluates potential candidates for admission, collaborating with patients, families, care teams, and community organizations to coordinate person-centered transitions. The role requires adaptability and responsiveness to evolving system pressures and patient needs, often supporting flow initiatives as they evolve.

 

Skills and Qualifications Required:

  • Graduate of a recognized school of Physiotherapy
  • Active, unrestricted license to practice with the College of Physiotherapists of Ontario
  • Experience with electronic patient record systems
  • Two (2) years of experience in providing clinical practice in a hospital setting with the relevant population. Intake and discharge planning functions, preferred
  • Demonstrated understanding and alignment to current healthcare funding models, expectations and deliverables of Providence Care Hospital, including achievement of efficient patient flow and targeted lengths of stay
  • Valid Ontario Driver's license, clear Driver's Abstract and access to a reliable vehicle as necessary

 

Position Responsibilities:

  • Leads Bed Utilization Meetings and other key meetings, as assigned, to facilitate communication and coordination related to patient flow
  • Reviews Daily Dashboard and clarifies data quality issues
  • Triages inpatient referrals for appropriate and timely review by respective inpatient Access to Care and Transition specialist
  • Problem solves barriers to transitions in care at admission and transitions in care
  • Provides daily and weekly summaries to Providence Care leadership
  • Review trends, variances and reasons for variances for admissions, transfers and discharges
  • Ensures appropriate and accurate data entry in support of safe transitions
  • Provides real-time resolution of issues brought forward regarding barriers to admissions, discharge refusals, repatriations and community support services
  • Facilitates community referrals to support efficient and effective discharge planning and/or transition as necessary (this includes but is not limited to community agencies such as Home and Community Care, Addiction and Mental Health Services)
  • Takes a primary role in managing patient/client/family/provider expectations concerning admissions and discharges and communicating discharge logistics to achieve timely transitions, positive outcomes, service efficiency and effectiveness
  • Facilitates access to care and transitions by actively supporting predictive discharge planning, where applicable
  • Acts as liaison and assists clients and staff with coordination of Home and Community Care and other community resources/services
  • Builds and maintains linkages with appropriate community agencies to facilitate effective collaboration related to access to care and transitions
  • Provides consultation to team members related to client care issues and community support services
  • Collaborates with team members in shared planning, review and decision making, to achieve optimal health outcomes for patients/clients and their families
  • Participates in profession-specific and interprofessional activities that are client centered and contribute to quality and safety, and operational efficiency, while promoting professional and program development
