Reporting to the Director of Patient Flow & Access, the Patient Flow Supervisor provides day-to-day operational oversight of patient flow activities, with a focus on supporting the Patient Flow Coordinators and Access to Care and Transitions (ACT) Specialists as well as guiding the Social Work team and advancing safe, efficient transitions of care. Through proactive identification of needs, system navigation, and the removal of discharge barriers, the Supervisor plays a key role in optimizing resource utilization, improving bed flow, and supporting alignment with organizational and provincial priorities. This role is also instrumental in coordinating efforts across teams and ensuring effective patient flow throughout Providence Care. This role is essential in ensuring efficient, high-quality, and seamless transitions of care across Providence Care.
Education and Experience:
- Degree in a Health Care Discipline, preferably at a Masters’ level; an equivalent combination of relevant education and experience may be considered
- Current unrestricted registration with the Ontario professional college of discipline, or eligibility to obtain
- Minimum of five years of clinical experience in a hospital setting, preferably with a focus on intake and discharge planning for the relevant patient population
- Experience with electronic patient record systems
Position Duties & Responsibilities:
- Supervises daily activities related to patient admissions, internal transfers, and discharges to support safe and timely transitions of care. Follows and helps to lead directives from Ontario Health pertaining to Patient Flow (ie Home First Refresh).
- Serves as a point of contact for the Access to Care and Transitions (ACT) Specialists providing informal guidance and supporting consistent practices across the team.
- Provides day-to-day operational support and guidance to the Social Work team on behalf of the Director of Patient Flow and Access, advocating for their operational needs and facilitating the resolution of discharge-related barriers.
- Chairs and leads “At Risk Rounds” to proactively identify and resolve complex discharges with interdisciplinary teams.
- Promotes early identification of patients requiring discharge planning to enable timely, coordinated interventions.
- Collaborates with internal and external teams to facilitate person-centered discharge planning and transitions of care.
- Works closely with the OHaH Manager of Patient Services to manage ALC designations, ensure appropriate sign-off, and communicate bed matches, offers, and declines.
- Reinforces and educates on the Home First Philosophy in accordance with Ontario Health directives, prioritizing discharge home with supports before considering long-term care placements.
- Assists the teams with repatriation planning to home hospitals when appropriate.
- Collaborates with interdisciplinary teams to troubleshoot and resolve flow-related barriers, both internally and with external partners.
- Leads and/or assigns Bed Utilization meetings and facilitates other key flow huddles, promoting communication and shared planning across departments.
- Supports ACT Specialists/Access and Social Work team with problem-solving for complex transitions or community referrals.
- Participates in escalation processes by communicating real-time flow challenges to internal leadership and external partners as needed.
- Assists the Director with compiling patient flow data, generating reports, and providing contextual information to support operational planning and quality improvement.
