Reporting to RAI Co-coordinator/ Supervisor, the RAI Assessor will primarily complete and coordinate the collection of information related to the Resident Assessment Instrument-Minimum Data Set –Lon-Term Care (RAI-MDS-LTC).  Working collaboratively with the Clinical Team, the RAI Assessor will ensure the service has access to and analysis of, any information available through the collection of residents’ assessments. Additionally, based on this analysis, the RAI Assessor will work with the Clinical Teams to provide recommendations for ongoing quality improvement of outcomes as well as support the evaluation and decision-making to maximize the value of care and services provided. 

 

Skills and Qualifications Required:

  • Diploma in health related discipline or field; Degree preferred
  • Minimum of 3 years’ experience in a health care setting using Continuing Care Reporting Systems (CCRS), required
  • Minimum 1 year recent experience in working with RAI-MDS data or similar clinical data set from another sector, required
  • Experience completing and applying RAI-MDS assessment data and information into daily clinical practice, required
  • Experience with performance measurement and quality improvement methodologies, preferred

 

Position Responsibilities:

  • Consults and collaborates with Clinical Teams
  • Completes RAI-MDS assessments as required by legislation
  • Ensures that key personnel (primarily nursing, allied staff members and also physicians) are aware of their designated tasks and have the needed support and resources
  • Supports specific training to all staff completing the RAI-MDS on data elements relevant to each specialty or discipline
  • Supports general training and education regarding RAI–MDS assessment and clinical application to all clinical staff, managers, etc.
  • Provides support to inter-professional teams to review RAI-MDS data, facilitate effective use of the data to support recovery focused resident care and resident/family collaboration in treatment and discharge planning
  • Consults and collaborates with clinical teams and site experts to encourage and facilitate the clinical utilization of data
  • Maintains open communication with clinical leadership and brings forward improvement initiatives in a timely manner
  • Actively promotes the use of the RAI –MDS in clinical practice
  • Assists the Program Managers and staff to plan, coordinate and evaluate resident plan of care
  • Participates on Quality Teams and other related groups, as requested
  • Supports audit processes according to established frameworks
  • Performs other duties, as required
